Period of Assurance

  • Yearly Coverage (rates subject to claims review)

Employee Entry Age (age at next birthday)

  • Min: 18 years old
  • Max: 60 years old

Employee Expiry Age (age at next birthday)

  • Max: 65 years old

Employee with Family

  • Legal Spouse: 18 years old to 65 years old
  • Unmarried children: 30 days to 19 years or 23 years (full-time student)
  • Employee must participate prior to the entry of spouse/children

Minimum members to be insured

  • Minimum of 5 employees
