  • Business Registration Form 9, 24 (full set) and 49, Trading License (if applicable), Form 13 (if applicable)/ Section 51, 58, 78 or other any related documents
  • Latest 2 years certified true copy audited financial statements
  • Latest 6 months’ company bank statements
  • Company's memorandum and article of association
  • Director and Shareholders copies of NRIC (both sides) or passport
  • For Corporate Shareholders to provide item No.1 to No.5
  • For Expatriates - Valid Work Permit (more than 6 months)
  • Credit Bureau consent letter
  • Employment confirmation letter

The Service Tax of RM25 governed by the Service Tax Act 2018 shall be imposed on each Principal and Supplementary Credit/Charge Cards upon card issuance and card anniversary effective 1 September 2018.
