Are you a sole proprietor / partner / professional body / incorporated or a private limited company?
Here are the required documents you’ll need to provide with your application.
For Incorporated or a Private Limited Company:
A photocopy of Memorandum and Article Association form (M&A), Form 9 or 13, Form 24 & 49 (full set from date of incorporation/registration), latest 3 months of Bank Statement and photographs of the company.
For Sole Proprietor / Partnership / Professional Bodies:
A photocopy of the sole proprietor or partners' Identity Card, Business Registration Form and valid license and the owner's valid practicing license or certificate from professional bodies (if applicable).
Please include a copy of your bank statement for the last 3 months and photographs of your business premises.
Do note that your business must be based in Malaysia.
