First are you a Malaysian or an expatriate with a minimum income at least RM60,000.
As a principle cardholder are you between 21 to 65, and are any supplementary cardholders at least 18?

For any Supplementary Cardholder all you need to prepare is a copy of the NRIC (both sides) or Passport.
As a Principal Cardholder, if you are salaried employee please prepare:
- A copy of NRIC (both sides) or Passport, including that of supplementary applicant’s
- Your latest 3 months' salary slips or latest EPF Statement
- Your latest BE form with official receipt
If you are self-employed please prepare:
- Copies of your Business Registration
- Your latest 6 months' Bank Statements
If you are an expatriate please prepare:
- A letter from your employer confirming the duration your of employment contract in Malaysia
- Note: You must be a Maybank account holder
If you are Graduates
- Copy of Degree Certificate/Professional Qualification
- Employment letter OR 3 month salary slip
If you are an existing cardholder
- A copy of NRIC (both sides) or Passport, including that of supplementary applicant’s
- Copy of Credit/Charge card (both sides)
- Copy of latest card statement
